Nephrology E/M?

I work in a Nephrology practice that sees patients in the office. Most of the time the patients don't have any symptoms for their CKD other than abnormal lab results. How can you use the 95 HPI guidelines since there usually aren't any symptoms until the CKD has progressed to ESRD or would you need to exclusively use the 97 guidelines?
