Substance Abuse Facility/BioFeedback

The substance abuse facility recently put in a biofeedback bed.
The client uses the bed for apx 60 minutes a session.

i am billing with a UB04 form,
rev 0917, 90876


All claims are coming back denied.

Any advice on billing errors?
