Coding help on several RF procedures

I have a new pain management physician that wants to perform the following injections. These are all new types of procedures to our office so they are uncharted territory for me as far as coding goes. I'm guessing most, if not all, of these would be billed with the unlisted code 64999. Looking for feedback.
