MCR Physical Tharphy 10th Visit

Need some feed back on documentation for MCR Physical Therapy for MCR patients on their 10th Visit OR 30 days of scrip which one comes first.

Firstly will this be documented as re evaluation or its an assessment

If re evaluation do the therapist need to state re evaluation and time spent, however no billing for re evaluation, also should they documentation its 30 day of scrip/10th visit revaluation?

Re Evaluation (every 30 days or 10th) visit and bill a regular session for treatment provided and G- Codes with Severity modifiers.
