TCM and other services

When we see a patient within 7-14 days of a discharge but we bill for TCM (Transitional Care Management) on the 30th day of the hospital discharge. What date of service do we bill if we also provided other services (such as an xray or flu vax), would it be on the actual date we saw the patient (within 7/14 days of d/c or the 30th (TCM date)?

I believe we would bill two different dates of service ?

Flu vax and admin on the actual date

and TCM visit on the 30th day ..correct?

I've read three TCM updates since 2013 on CMS, but no mention of the additional services provided billing date.
