Uterine Specimen Weight if not weighed in OR

I need documentation/sources to support that the weight of the Uterus is NOT taken from the Path report. If the Physician doesn't weigh it at the time of removal the procedure defaults to the 58571 250 gram or less - even if the Path report says 280 gram etc. because we don't know what all the Pathologist weighed because tubes and ovary were taken at the same time.
In 15 years of coding I haven't ever taken a weight or size of lesion from the path report because it changes in the solution etc. just if Malignant or Benign etc. diagnosis. Thank you!!
