PEG Tube Removal - Outpatient

I know that if a PEG tube is removed in an office setting - it is appropriate to use the E/M code.

I have one that the doctor did at hospital / outpatient;

Medications - Fentanyl 50 mcg and Versed of 3mg

Indication - Malfunctioning PEG tube

Findings - PEG tube appeared to be clogged. This was originally placed push type Bard PEG tube, which was externally removable. After adequately sedating the medications, the PEG tube was pulled out without any complications. There was no bleeding at the end of the procedure. The patient tolerated the procedure well. There were no complications.

Impression -

Malfunction of the percutaneous endoscopic gastrostomy tube

Removal of the percutaneous endoscopic gastrostomy tube.

Any suggestions would be helpful

Thanks
