Nerve Block of L5, S1, S2 and S3

The physician says the procedure performed was a block of the L5 dorsal rami branch nerve with additional levels, lateral branches of S1, S2 and S3 (all on the left side). I have one physician who want to bill 64450 X4 and another who wants to bill 64493 x1 and 64450 x3. I did a search and was not able to find a definitive answer. What is the correct way to bill this?
