cementoplasty of left acetabulum via an anterior approach?

An 10.5 gauge needle was placed into the skin and advanced to the

level of the periosteum of the anterior aspect of the left

superolateral acetabulum via an anterior approach. The needle was

advanced to the mid to posterior aspect of the acetabulum and a core

biopsy was obtained. The cannula was then advanced to the posterior

aspect of the acetabulum. A Spine STAR 10/15 135 mm RF bipolar

ablation probe was then placed through the working cannula. Ablations

were performed at the posterior aspect of the acetabulum, angling the

tip of the probe both medially and laterally. The probe was then

retracted approximately 1 to 1.5 cm and additional ablations were

performed. A total of 6 ablations, each lasting approximately 4

minutes were performed. Two of the ablations had the proximal thermal

couple reach a temperature of 50 degrees Celsius, ending with a power

level of 2. Four of the ablations had the distal thermal couple reach

a temperature of 52 degrees Celsius and the proximal thermal couple

reach a temperature of approximately 47 degrees Celsius.


The working cannula was then advanced to the posterior aspect of the

acetabulum. Cement was injected, with serial CT scans acquired to

ensure that no cement extravasated into the left hip joint. The

working cannula was retracted several times to allow more anterior

cement placement. A total of approximately 10.5 cc of cement was

placed, with good filling of the lytic lesion.


The needles were removed and direct pressure was placed over the skin

puncture sites, and good hemostasis was achieved
