When multiple X-modifiers apply to a claim, would you use all three or which one would supersede the others? I code for pathology and many times their are two surgical specimens done same day. (i.e. colonoscopy done same day as bronchoscopy, different providers, different specimens, same cpt codes.
The XE, XP, XS all apply. How would it be reported 88305-26, 88305-26/X??
Modifiers X (EPSU)???
