minimum documentation requirements for in office procedures

I am looking for a source which lists the minimum documentation requirements for in office procedures performed both by a provider or his/her ancillary staff.

A note example I have is " the patient comes in for routine suprapubic tube change. He reports no problems". The note is signed by the MD who is billing a 51705.
