I was just asked to obtain pre-certification for Total Hip Replacement for ASC with no supporting documents (CPT codes). I have tried requesting a sample report from the surgeon?s but to no avail. As this is my first Orthopedic case, any assistance and feedback, including a sample report would be greatly appreciated.
