59 modifier and Orthopaedics 2015

Newer to Orthopaedics;

Needs some assistance with this new 59 modifier subsets:

Dr.s have asked the following questions, can anyone help?

Is there somewhere that I can go to learn the specifics for future scenarios

1. PA doing and H&P urgently the same day a patient is seen by the physician

2. Does this mean patients can now see more than one provider per day, if proper 59 code subset is used (eg: Dr. visit and PT)?

3> Is is Medicare patients only?
