Quality Incentive Coding

Does anyone know what procedure code they use for the "Self-Management Goals." I understand this is an incentive qualification. The patient (and provider) discuss and document the goals the patient sets for him/herself. We were told to use 99199 but that is an unspecified procedure code and not sure what notes or documentation would need to be included. We are not charging for this procedure.
