Billing 92250 to Medicaid

I have a denial for CPT code 92250 with ICD-9 code 365.01. This is a covered diagnosis. I spoke to a representative at NC Tracks and she said that maybe there was a better code to use. I do not see anything that states I must code first or use an additional code with it. Has anyone else had this denial and what did you do to reimbursement?
