I know that if a patient is seen in office and then admitted w/o face to face documentation for the hospital that for this dos you would need to bill the E/M code for the OV. My question is that on the following day when the patient is seen in the hospital for the first day, I have been told I can bill an admit, but what documentation is needed? Does the doctor need to document another H/P besides the one from the day before for the OV ?
