ER Consult

Question about an ER Consult for a Medicare patient:

-Orthopedic physician was called in by the ER provider to consult a patient.

Consult performed and surgery needs to be done; however documentation

states the patient is being admitted to the Hospitalist for management of

medical conditions prior to surgery.

My questions is should this be coded as 99284 (E/M elements support level 4) or 99204 since the patient is being admitted, but not by the consulting physician?
