I've only seen older posts regarding this that say that Harvard Pilgrim wants -59 even though it is an add-on code; and Secure Horizons paid for PA assist on 3rd party review. I didn't even think of charging for the PA assist, but it isn't on 2015 MC list of procedures not payable for an assist.
