VAD Subsequent Inpatient Billing by CT Surgeon AND Cardiologist

There has been discussion in our CT Surgery practice regarding billing subsequent inpatient visits of VAD patient's by both the CT Surgeon and Cardiologist. How is this handled in other Practices? What does each specialty's note look like?
