NP billing

I have recently been given the responsibility of billing for our new NP. She sees patients in a SNF. She wants me to bill for visits in following ex: pt seen on 7/2 for regular care bill 99308 then saw again on 7/4 for removal of impacted cerumen bill 69210 -50 for bilateral plus another 99308 with-25 mod, then saw again on 7/6 for follow-up care. I am aware that just she documents medical necessity she can see the patient for all three visits. My thoughts are bill first visit 99308, second visit 69210-50, and the third would be included as part of global period of the second visit? Would this be correct? Any advice appreciated.

MCR Physical Tharphy 10th Visit

Need some feed back on documentation for MCR Physical Therapy for MCR patients on their 10th Visit OR 30 days of scrip which one comes first.

Firstly will this be documented as re evaluation or its an assessment

If re evaluation do the therapist need to state re evaluation and time spent, however no billing for re evaluation, also should they documentation its 30 day of scrip/10th visit revaluation?

Re Evaluation (every 30 days or 10th) visit and bill a regular session for treatment provided and G- Codes with Severity modifiers.

Thanks

question about coding and hip surgery

I have a bill that shows the following CPT codes used on the bill, this is all the same surgery

29914

29915

29862

29916

29999

29861

It seems like they listed everything they did but shouldn't some of these procedures be bundled and not listed separately? Along with each code there is a cost charged. I am a coder but I do not do coding on a regular basis and need help.

Thanks Carol Orazi, CPC

Billing for physicals when the GYN physical done elsewhere

We are a family practice. Several of our patients schedule for physicals. When the doctor goes in the room, the patient states they had their GYN exam elsewhere and just want the physical. What codes are billed out by GYN physicians, and what codes are appropriate for the family practice to bill out?
